The agency head may appoint an appropriate professional if he or she chooses not to serve as the Ex Officio Trustee. Forms for designation of the Ex Officio Trustee are available through the Field Services Department of APH. These forms must be signed by the official charged with appointment of the Trustee (see listing above). Prompt notification to APH of changes in trusteeship is essential to ensure continuing and effective service.
